CYP4F2 (Cytochrome P450 Family 4 Subfamily F Member 2) encodes an enzyme with important roles in both vitamin K and arachidonic acid metabolism. CYP4F2 catalyzes the omega-hydroxylation of vitamin K1, effectively inactivating it. The enzyme also produces 20-HETE from arachidonic acid, a potent vasoconstrictor and regulator of kidney function.The V433M variant (rs2108622) reduces CYP4F2's ability to metabolize vitamin K, leading to higher hepatic vitamin K levels. Carriers of this variant require higher warfarin doses to achieve therapeutic anticoagulation. This variant is included in FDA-approved warfarin dosing algorithms alongside VKORC1 and CYP2C9. The same variant may also affect blood pressure and cardiovascular risk through altered 20-HETE production.
