CYP2D6 (Cytochrome P450 Family 2 Subfamily D Member 6) encodes one of the most clinically important drug-metabolizing enzymes. This enzyme is responsible for metabolizing approximately 25% of all clinically used medications, including antidepressants, antipsychotics, opioids, beta-blockers, and tamoxifen.CYP2D6 is highly polymorphic, with over 100 known allelic variants resulting in four metabolizer phenotypes: poor metabolizers (PM), intermediate metabolizers (IM), extensive/normal metabolizers (EM), and ultrarapid metabolizers (UM). Poor metabolizers may experience drug toxicity at standard doses, while ultrarapid metabolizers may have reduced drug efficacy or, in the case of prodrugs like codeine, potentially dangerous rapid conversion to active metabolites.
