Red Yeast Rice (RYR) in the Treatment of Cardiovascular Conditions and Hyperlipidemia

Red Yeast Rice (RYR), a traditional Chinese medicine, has been studied for its effectiveness in treating carotid atherosclerosis. A comprehensive systematic review and meta-analysis of 20 randomized controlled trials involving 2217 patients showed that RYR significantly reduced carotid plaque area, thickness, and score, improved lipid profiles, and presented an acceptable safety profile. The treatment also positively affected low-density lipoprotein cholesterol (LDL-C), triglycerides, and total cholesterol levels, while increasing high-density lipoprotein cholesterol (HDL-C). Despite some study heterogeneity, a dose-effect relationship was observed, confirming RYR's efficacy and safety for the Chinese population.

Another meta-analysis focused on RYR extract in myocardial infarction (MI) patients with borderline high cholesterol. Analyzing data from seven studies with 10,699 MI patients, it was found that 1,200 mg of RYR extract daily significantly reduced the risk of nonfatal MI, the need for revascularization, sudden death, and improved lipid profiles.

A separate meta-analysis compared the efficacy of RYR against statins and other nutraceuticals in treating hyperlipidemia. With 15 randomized controlled trials and 1,012 participants, RYR was found to be more effective than statins in lowering triglycerides and equally effective in managing LDL-C and HDL-C levels. It also showed promising results in combination with other nutraceuticals for improving lipid profiles and was deemed a safe treatment option.

In assessing the benefits of RYR on hypertension, a systematic review of 21 trials with 4,558 patients indicated that RYR, in combination with conventional therapy, could significantly lower total cholesterol, LDL-C, and CRP levels but had a limited effect on blood pressure and triglycerides. The combination of RYR and conventional therapy had comparable results to statins plus conventional therapy.

Lastly, a research article reviewed the traditional uses, chemical composition, pharmacological effects, and safety of RYR. It highlighted the diverse health benefits associated with RYR's constituents, including monacolins, pigments, and organic acids, suggesting benefits for cholesterol management, atherosclerosis prevention, and other health conditions. However, the article emphasized the need for further research to explore the full spectrum of RYR's bioactive compounds and its traditional applications.
